I was, for a time, a slot attendant at a casino in Atlantic City. If any single payout was $ 1200 or more, we had to issue a W2 with the winnings. Customer had to have SSAN on file. With a player card this is already set, but if the customer had no player card they had to show ID with the SSAN. I saw it happen that one lady had no ID so the supervisor issued a voucher to her and she had to return later with ID to get her money.
In New Jersey at least there is no withholding unless the customer requests it. I can say that in New Orleans the casino withholds Louisiana state tax. My wife and I found that out the hard way on our last trip after drawing a royal on a progressive machine for $ 1800!

The last time that I hit a taxable winner in Vegas (3 years ago) I wasn't even given an option on taxes - none were taken out. I don't know if that was just the policy at that particular casino or a change in NV state gaming regs. I hit several big winners that trip and was never given the option, even though I asked about it. Before that I was always given the option.
