This is what I track:

Date
Casino
$, Won/Lost
$, Cash Comps
$, Non-cash Comps
Session Start Time
Session Length
Time Played, by Denomination ($.05 - $5)

For each nice hit I get, I make a note of:

Time, rounded to nearest minute
Type (5's-K's, bonus quad, Aces, kicker or no kicker, etc.)
Game (DB, DDB, BP, Deuces, etc.)
Denomination

I take notes on my PDA, using an application I wrote some years ago and that simplifies my notetaking - the application saves my session info in .CSV format.  When I get home, the PDA gets plopped into its cradle and I copy the session file to a new tab in my yearly play spreadsheet model.  I then run a macro that links the new info to different places in my model.

Thanks for the great replies! I noticed that you are tracking your comp $.  Are you tracking what is displayed on the machine?  Or are you calculating the comps on your own?  Also can you clarify "session"?  Is a session based by machine or game? Or bankroll allotted by day?

For example,I have a daily bank roll of $500.  If I sit at a DDB game and put in a Franklin.. and not do too hot, losing the whole $100.. Then next I go to a FPDW and put in $20, I lose the $20 but I decide to stay at the same machine and put in another $20.  Would this be 3 different sessions?  How granular do you get when machine hopping or feeding the same machine?

I start a new session if I ...

1) Change denom
2) Change game
3) Change casinos
4) New day

Some people track machines as well but I don't.
