The situation you bring up is the American Coin incident in Nevada (not A.C.). The man killed was Larry Volk. You can read more about it here:

Since that situation the Nevada gaming commission requires all manufacturers to provide source code for their games. That should make situations like it much less likely as it would take a conspiracy of sorts.
