Watching the video it is an obvious malfunction. That being said, in this day and age we shouldn't have these crazy malfunctions -- maybe there are a lot of little malfunctions that no one notices?

This has happened before a couple of years ago, and casinos have taken all precautionary measures to protect themselves in the event something this happens again.
