$40,000 dealt Aces w/ kicker at Harrah's Cherokee
This was Frenzy Poker, but that's secondary -- Triple Double Bonus, dealt 10 hands, AAAA2 -- $40,000!
$60 bet.
Best ever. Miracles do happen.
But this game is highly volatile. If you don't get a reasonable frequency of dealt three of a kinds, forget it.
Pay table on that Triple Double is 45 for a full house, 25 flush, 20 straight, 10 three of a king, 5 and 5. Not sure what the payback percentage is, maybe someone can chime in.

To answer your question about the return, my Video Poker Pocket application (made by videopoker.com) says 97.22%. The IGT media guide that Wizard of Odds Shackleford received says 97.36%

I saw a $17,000 slot machine hit in Caesars high roller room a few weeks ago. That blew me away. But especially for VP, this is remarkable!
